is that good enough to go to the new dr this wednesday? i keep calling dr office but the answering service is always there. i have an appointment in 2 days, and the new dr needs a release of medical records , would that paper be it? they also want me to sign a paper when i get there that they can access my medical records…..really confused never switched a dr before and this is 11 weeks into pregnancy

called old ob finally got through, she said write a paper out w/ all my info and who they are releasing info to and fax them and thats that

go to your old obs office and request the paperwork in person….that way if you need to sign anything you can go ahead and do it and they will forward the info to the new obs office……also, be prepared to pay for any of these documents from your chart…..it can be quite expensive
The new doc should provide you with a form for you to sing allowing your old doc to release your info to them and only them,you sing it and they will fax it to the old doc, in turn the old doc will fax all you medical records and that’s all that should be needed
